Who is Telehealth For?
Our virtual services are available for all age groups, from children to seniors. Telehealth is the ideal choice for addressing illnesses such as flu, cold, sore throat (strep throat), earache, sinus infections, seasonal allergies, eye infections, rashes and skin lesions, bug bites, stomach/GI symptoms, minor musculoskeletal concerns, infections including UTI & STD, immunizations & lab testing, primary care follow-ups, and many more. DO YOU OFFER IN-PERSON APPOINTMENTS?
All of our services are offered in an innovative, convenient, and secure telehealth environment. Most routine health concerns can be handled without ever seeing a healthcare provider in person.
WHAT IF I NEED A REFERRAL?
Should you require a referral to an in-person healthcare facility, we will not only connect you with an excellent provider, but work diligently to get you in quickly. We maintain close relationships with premier providers around Tarrant County that handle all types of health concerns and we are commonly able to get patients seen within 24 - 48 hours.
WHAT IF I NEED LABS, IMAGING, OR PRESCRIPTIONS?
We maintain relationships with Quest Diagnostics for lab services and Midtown Imaging for imaging services. It is common for patients to be able to be seen within 24 hours at a convenient location near you. We also have the ability to digitally transmit prescriptions to your pharmacy of choice.
DO YOU ACCEPT INSURANCE?
Momentum Family care is an innovative practice which affords patients many conveniences and benefits not common of today’s medical practices. Momentum Family Care does not bill insurance. Patient do not pay copays for sick visits nor incur surprise charges for simple testing. There is no need to meet a deductible or get insurance approval for visits. You may submit your visit to your insurance for reimbursement (insurance dependent).
How Do I Pay?
We accept all major credit cards, ApplePay, CareNow, FSA, and HSA. Payment for scheduled services will be collected prior to services being rendered.
IS THE APPOINTMENT SECURE?
Yes, all appointments and communication are HIPPA compliant. All necessary forms will be filled out and completed prior to your first appointment. Once you are patient, you will have access to your medical records, labs, referrals, medication history, etc.
CAN I GET A COPY OF MY MEDICAL RECORDS?
Absolutely. You will have access to your medical records, labs, referrals, medication history, etc. via our patient portal at any time.
